The patient is a 72 year old woman with a shave biopsy of a pearly, telangiectatic, 7 mm papule on the right medial superior chest. Clinical Diagnosis: BCC vs irritated SK vs SCC.

Yes, Follicular porokeratosis. I saw one nice case recently in consult that was previously diagnosed as sebaceous hyperplasia. Rare variant of porokeratosis.

Yes, you are all correct.

SKIN, RIGHT MEDIAL SUPERIOR CHEST , SHAVE BIOPSY :
[b]-- INFUNDIBULAR ("FOLLICULAR") POROKERATOSIS[/b]
[indent=1][b]COMMENT:[/b] This is an unusual variant of porokeratosis that is confined to the infundibula. In some patients reported in the literature, it can affect the surface and the infundibula.[/indent]
