F45. 4yr history of papule right upper lip. Small puncture present but poorly BCC ? Cyst ? BCC.

Pilar sheath acanthoma with and without granular layer, maybe the tumor developed near the junction between the isthmus and the infundibulum that is why we have this hybrid features

I think this could be coming from oral mucosa...many diskeratosis...Favor Verrucous carcinoma

No tricks today.  Just a nice example of a (typical) pilar sheath acanthoma. These are rare in my experience. I posted it for you to contrast with case 1495 which is far from typical (an odd hybrid lesion) although I suspect largely of academic interest / discussion.
