I thought it was hyperpigmentation secondary to levofloxacin there is massive actinic elastosis (sun exposed area) and golden pigment positive with perls

Agree. Like Raul, I too think that this is Levoflox induced hyper pigmentation. Positive Perls. Massive actinic elastosis.

Drug induced pigmentation caused by Levofloxacin. It is rare but well described and the pigmentation stains positive with Perls.
