What about verruca vulgaris with trichillemal differentiation (possibly long standing lesion)

I see areas in picture 2 and 3 with parakeratosis, presence of granular layer, papillomatosis and congestive papillary vessels typical of verruca. Trichillemal changes can be seen in verruca and apparently some authors supported the theory that trichillemmoma is derived from ancient verrucas. I accept that the changes are more than focal but I still see part of a verruca.

Trichilemmoma. There can be overlap with VV and I think Dr. Ackerman's arguments have a lot of merit!
