Scabies, patient must be immunocompromised to have such heavy infestation. Was he erythrodermic? Could be Norwegian scabies.

The infiltrate appears neoplastic.  Looks plasmacytoid.  Reminds me of a case of Norwegian scabies in a man who had ATLL many years ago when I was a neophyte - lovely pop-corn cells in that case.

Agree with norwegian scabies and non-Hodgkin lymphoma. There is a known association of norwegian scabies and adult T-cell leukemia/ lymphoma.

The infiltrate does look a bit worrying. Had missed looking at it carefully first! There are some large cells, in a mixed population. Some of the cells appear myelomonocytic and some degranulated mast cells. Not sure about others.

I missed the infiltrate too, looks leukemic :-)... I suspected immunosuppression from the heavy infestation!!
