Case History: 44 year-old Hispanic male with punch biopsy of lesion from lower back. Clinical impression: rule out vasculitis. Prior history of scleroderma.

Bullous vasculitis with epidermal necrosis, vessels with fibrin thrombi. Special stain shows 'globi' of acid fast organisms. Erythema necroticans/ENL.

Agree with erythema nodosum leprosum. All pathological findings fit: neutrophilic infiltrate, vasculitis with trombosis, panniculitis, ischemic epidermal necrosis(Lucio phen)...

Only the site is atypical. We can define it leprosy with perineural and perivascular infiltration, trombosis and Lucio p.
