Agree, Trichoblastoma. Listen, SOMEBODY COULD HELP ME? SPOT DIAGNOSTIC IS FREE, BUT I PAID A FULL MEMBERSHIP SUBSCRIPTION TO STUDY AND THE ONLINE COURSES, SPECIAL MODULES AND SOME VIDEOS AREN´T WORKING. I wrote to the website administrator a couple of times but don´t have answers yet.

My impression is there are many dendritic melanocytes (pic3/4), so I would like to name this Melanotrichoblastoma...what do you guys think about?

I think that if HMB45 shows hyperplasia of melanocytes I would agree with you and propose to publish it my friend (if the case was ours of course)

Agree with the diagnosis of pigmented trichoblastoma associated with hyperplasia of melanocytes.

Thank everyone for the insightful discussion. I agree with everyone that this is a  benign neoplasm with follicular differentiation.  I am mostly impressed by the recapitulation of the follicle matrix -- so I made the diagnosis of melanocytic matricoma. I'm not aware of a set of criteria that can reliably distinguish it from trichoblastoma. In my opinion they are in the same spectrum. I'm open to any advice. Thanks!

Dear Limin,

I have a collection of melanocytic matricomas and they don't have papillary mesenchymal cells and do have matrical keratin and usually are diffusely Beta-catenin positive. Your case looks like a typical example of a pigmented trichoblastoma to me.
