I had the same feeling as Mona, I am worried about the cells in the epidermis, maybe some pagetoid spread of sebaceous carcinoma. Would definitely perform immunos in this case.

I can discern intraepidermal scatter of cells with ample multivacuolated cytoplasm. Agree with Mona and Raul, this is pagetoid spread of periocular sebaceous carcinoma.

Sebaceous carcinoma in situ.  Adipophilin was focally positive.
