Angiosarcoma. The history is interesting. 90 year old man with smaller patch on temporal scalp called 'benign' and watched for two years. The patch continued to grow and a second patch appeared; the first patch was then biopsied and yielded this biopsy. Issues here include informed consent (patient has moderate dementia), difficulty in making an 'early' diagnosis of angiosarcoma, and treatment options in someone in this age group.
