Acantholytic Bowen Disease

Agree with above. It could be a Grover disease, with epidermal dismaturation pattern

I’m going with Grover’s disease based on clinical, relatively mild atypia and no solar elastosis (that I can detect).  Corps ronds / grains would be reassuring but maybe they’re seen on deeper cuts. The extension down a follicle is helpful.  At most, the atypia could push me to an eroded/acantolytic AK but am holding off.

I share similar impression as Alice. I favor Grover's disease, especially with the clinical description of "rash". However if clinically this is a solitary lesion then the classification becomes very challenging to me, because acantholytic actinic keratosis cannot be excluded.
