Interstitial granulomatous dermatitis (IGD).You may see  ('floating sign') .

I reported it as in keeping with granuloma annulare (incomplete type) with subtle granuloma-like collections. We've discussed previously that there are rare case reports of patients with both GA and sarcoid. I probably mentioned this in reporting the case. There was a picture of a similar case, brought to my attention by Dr Saleem Taibjee, pictured in Akerman's original classic (gold) book "Histologic diagnosis of Inflammatory Skin Diseases". The image did not re-appear in the subsequent 2nd Silver or 3rd Gold/Silver editions.
