I see a verrucous lesion with viral cytopathic changes and endophytic, pushing growth. I think it is Buschke-Lowenstein tumor (giant condyloma). Needs complete excision to remove the lesion and make sure that there is no deeper, true invasion.

Difficult. I go for viral changes so I agree with Giant Condyloma and also I see some areas of High-grade dysplasia.

I wonder if HPV typing in this specimen can help. It can be a rather inexpensive test.

Thank you for the great discussion.  This is a verrrucous tumor with poroid features and high grade dysplasia. It's a porocarcinoma.
