Eosinophilic spongiosis with vesicle formation and underlying superficial and deep somewhat wedge shaped mixed lymphocytic and eosinophilic infiltrate. Possible flame figure? Most compatible with an arthropod bite reaction.

Such severe insect bite reactions (atleast in India) are seen in patients with HIV or hematological malignancy. Is there any such underlying condition?

Yes, mostly insect bite. Agree with Anil's concerns
