Hyperkeratosis, hypergranulosis, irregular acanthosis along with basal cell degeneration and lichenoid infiltrate in the dermis. I think this is hypertrophic LP.

My first thought was LP but then realized that there might be a dermal lesion. My overall impression is dermatofibroma. The lichenoid inflammation is only focal. More photos of the dermal portion might be helpful.

Thanks for the insightful discussion. This is a DF. Its paucicellular nature is not easily appreciated in images.
