An epidermal cyst with papillated lining and coarse, irregular keratohyaline granules. Looks like a verrucous epidermal cyst (HPV related).

Agree with verrucous follicular cyst but I think this one clearly hybrid infundibular-tricholemmal. They are HPV associated. I'm not aware of any specific connection with HIV. Tricholemmal cases (of verrucous epidermoid cyst) are rare. I advised checking that patient is up to date with cervical screening and should be checked for other HPV associated lesions.
