27 year-old HIV-positive male with widespread lesions all over his body. This biopsy is from an unspecified site.

Agree.  I hadn’t seen any in HIV before. And would like to see a HHV8 also. There are some slit like vessels between eccrine glands.

NEH was my first thought, too but upon further scrutiny I'd also like to run GMS to make sure that there are no organisms, such as crypto.

Histoplasmosis, best appreciated on the bottom image. Please see the GMS below.  The patient was found to have disseminated histoplasmosis.

Nice case, also thought of neutrophilic eccrine hidradenitis at first spot but as a rule in immunodepressed setting should rule out bugs
