51 year-old female with end stage renal disease, on warfarin. This is a right lower leg biopsy.

Epidermal necrosis, thrombi in dermal vessels and hemorrhages in the dermis- this is coumarin necrosis

Warfarin-induced calciphylaxis.  Serum calcium and phosphate levels were normal.
