My exact differential diagnosis. Sebaceous carcinoma vs sebaceous adenoma vs sebaceoma. The lesion was overall well circumscribed and did not have infiltrative borders. There were areas of well differentiated sebocytes. The architecture of a sebaceous adenoma was not present, so our final diagnosis was sebaceoma. However, the lesion had a crateriform silhouette that has been described in cases of Muir Torre syndrome. So, while we favored sebaceoma, we thought the patient should be worked up for Muir Torre syndrome.
