I would go with a bleeding in a lymphangioma or a lymphohemangioma.

Angiokeratoma is a good suggestion if it looks circumscript & vascular clinically.

However this looks lymphangiomatous. Lesions such as this can be seen in the setting of chronic lymphoedema.    
Underlying possible aetiology includes Crohn's disease, hidradenitis, previous surgery, DXT     
and morbid obesity in some cases.  Is there any relevant previous history?
