The top of my differential diagnosis is a metastasis, particularly from the breast. Once I can completely rule out that possibility, I will start thinking.

First spot: metastatic lobular breast carcinoma. 

Difficult to think in another differential than Metastatic Carcinoma (also agree that my first possibility would be a lobular breast carcinoma)

Yes, we were quite concerned about metastatic breast carcinoma and could not rule it out based on the histology. It's a fairly recent case and I have no further information. I have not seen any skin adnexal tumors look quite this way. The tumor is CK7 positive and CK20 negative.

If you do GATA-3 and ER/PR, that should nail the diagnosis of metastatic breast carcinoma.
