If it weren’t for increased dendritic network would have called it Paget’s

Yes, first impression: Extramammary Paget´s disease with colonization of dendrit melanocytes.

Differential include: Melanoma and Pagetoid SCC (least likely)

Yes, agree! We can call this "Pigmented Extramammaty Paget Disease"
