Follicle-centered granulomas in facial region--->?Rosacea.

Granulomatous rosacea is certainly a possibility as Vincenzo has suggested. Periappendagial granulomas are also a feature of leprosy.

I thought about granulomatous rosacea, too. But I am also impressed by the increased stromal mucin. I wonder if this can be lupus. Rosacea usually have sebaceous hyperplasia (which is lacking in this case) and telangiectasia (which is not prominent in this case). But then, I don't know how lupus can have granulomatous areas...

Overall probably rosacea. But would like to know if the patient clinically has rosacea like changes in other areas of her face.

Yes, Granulomatous Rosacea is my first thought.

Just for the sake of the differential let add one more: Lichen Scrofulosorum.

Or maybe is Rosacea in a patient with pre-existing Lupus.

dermal vague granuloma near pilosebaceous structures

? rosacea

? Hansen-special stain for AFB

Dear All

 

this is an interesting case as the clinical was that of ? Lupus miliaris disseminatus faciei (acne agminata) and this was the best fit at the CPC !
