No comments yet, perhaps suggesting that, like me, everyone else is finding this one difficult.  There seem to be two histological components.  In the papillary dermis there are nests of somewhat epithelioid cells with some pigment suggesting a junctional Spitz naevus.  Beneath this is a heavy lymphoid infiltration.  Within this there are areas of larger cells possibly hinting at follicle formation, or maybe these cells are related to the Spitz feature.  Perhaps this is a pseudolymphomatous reaction, but how it is related to the Spitz-like features I do not know.  I look forward to seeing what pathology colleagues think.
