considering the age of the patient can we have IHC

 DT is CK20+, AR+ while MAC is usually negative for these markers.

Dr. Chaudhry final comment on the case - please see below;

 

Dear All - Thank you for your comments. 

Our differential diagnoses were MAC versus DT versus infiltrative BCC. The tumour was BerEp4 positive and CK 20. We did. It do not perform androgen receptor markers. 

The final differential was DT versus BCC. The clinician felt it was the latter and he was to perform MOHs.

Looks typical of DTE. Demonstrating reactive MC with CK20 is useful (not seen in INF BCC).
