Trichilemmal differentiation and inverted follicular keratosis pattern, without convincing malignant features. Favor an Irritated SK, but would like see p16 and p53. 

Please, Dr Iskander: what’s the 2710 dx?

Thanks everyone - yes I agree with IFK. I think it has to be in the irritated group and a caveat to see we favour reactive over dysplasia as dysplasia has been described in these lesions.

squamous lesions are HARD. I guess the atypia is in the eyes of beholder...
