The nuclei are blunt ended and the second image shows perinuclear vacuoles, I favor leiomyoma

Mildly atypical...what about a low grade leiomyosarcoma?

I think these things are quite difficult. This particular tumor had enough atypia and mitotic activity that we felt comfortable with leiomyosarcoma. However, distinction among atypical lesions, so called symplastic lesions and true sarcoma is not easy and is in the eye of the beholder. The advantage of a slight overcall in this case is an appropriate, conservative excision.
