I favour sarcoidosis with not-so-naked granulomata.  Nice Demodex.  Would also consider granulomatous rosacea.

Early lesions of LMDF may not show frank central necrosis, and show only granulomas which may be sarcoidal or tuberculoid. Can be considered if multiple lesions, but in this case based on clinical suspicion of BCC, seems like the lesion is single.
