M28, Very long history (dating since early childhood as the patient recalls). Pruritic papules and nodules over both legs, some are crusted. Many lesions are studded with milia.

I think this is probably epidermolysis bullosa pruriginosa, a rare variant of dystrophic EB.  It is due to a mutation in COL7A1.

I´ve seen some cases of epidermolysis bullosa pruriginosa, but i never saw so much milium formation. It is a good hypothesis. Epidermolysis bullosa pre-tibial is another possibility.

The subepidermal reactive granulation tissue fits well this diagnosis.
