I think this may be another example from Saleem of BCC with neuroendocrine differentiation. 

I am assuming that the "elderly man" was not excised, only a small part of him!

Infiltrative BCC. There seems to be area of neuroendocrine differentiation.

I think my mission is partly accomplished! At least 2 people are now readily identifying BCCs with neuroendocrine differentiation which are not uncommon. The immunostains are fully compatible (BerEP4 positive, EMA negative, CK20 negative, chromogranin and synaptophysin positive). Unfortunately there is a glitch, and I can't upload the images. I'll ask the DermpathPro team if they can do this.
