Dense, dermal granulomatous inflammation with Langhans' giant cells and reactive epidermal pseudo-epitheliomatous hyperplasia.  Histology would do for mycobacterial infection or Crohn's disease.

Granulomatous inflammation..History, clinical correlation plus workup for infections (special stains and culture) is required .

Cutaneous Crohn's. The patient has Crohn's disease.
