Sarcoidosis but only after doing bug stains especially Fite to rule out mycobacterial infection- Hansen's disease.

Foamy cells are seen in the granulomas. I would put borderline lepromatous leprosy before sarcoidosis in the list of differentials.

Agree with above, would need to exclude infections (leprosy) , sarcoid lower in the differentials

This was sarcoidosis. MZN stain was negative. Chest radiological evaluation showed picture consistent with pulmonary sarcoidosis.
