The colour balance has been distorted by attempts at digital colour correction.

There is a small, superficial dermal tumour composed of islands of basaloid cells forming ductal structures set in a sclerotic stroma .  To my eye there is a degree of mild pleomorphism in the cells lining the ducts.  No epidermal connection is visible, nor keratocysts or calcification.

Desmoplastic trichoepithelioma is possible, but I am worried by the cellular pleomorphism.  I would also think of microcystic adnexal carcinoma, but it would be a small early tumour without all the usual features.

I'm fishing about a bit here!

I called this clear cell syringoma.  I apologize for the color - I am not sure what happened here.

Syringoma, basaloid islands with focal ductal differentiation and tadpole appearance, in a sclerotic stroma, the max is well circumscribed and not infiltrating, excluding MAC
