granulomatous inflammation in dermis, 

rule out infections and with other lab data rule out sarcoidosis

Non-necrotising granulomatous inflammation, I think there is possibly a Schaumann body. Favour sarcoidosis on morphology but need to exclude infective causes/foreign body impaction.

Classic sarcoidal granuloma, well defined naked granuloma occupying the upper and mid dermis,  with many multinucleated giant cells.
