Lichen striatus.  Adnexal involvement is supportive.

Monolateral, syringotropic lymphoid infiltrate, without atypia, and age fit well the Lichen Striatus diagnosis. CD8-positivity of intraepidermal lymphocytes would help.

since it is a rash i would consider unilateral latero thoracic exanthem that has a lichenoid infiltrate and eccrine infiltrate
