40 year old male with morbid obesity, shortness of breath, and rash of several weeks duration. Treated with antibiotics and steroids without change. Now in ICU with multi organ failure and worsening rash.
No new medications.

sub corneal pustule, would like to rule out AGEP

Agree with AGEP.  There is also considerable red-cell extravasation without obvious leukocytoclastic vasculitis, suggesting a capillary leak syndrome as part of multi-organ failure.
