F, 58 ,Progressive course of multiple hard cutaneous and sub-cutaneous nodules involving the scalp, nape and trunk of around 1 year duration. The skin condition is associated with difficulty in breathing. A recent chest X-ray revealed multi-focal patchy ill-defined ground glass opacities in different segments of both lungs. A biopsy was taken from one of the trunk lesions.

First spot: Mts of cribriform adenocarcinoma of the lung ( ttf1? ck7? ).

This will require extensive investigations. Is it a carcinoma of the lung metastatic to the skin or some other internal malignancy metastatic to lungs and skin needs to be considered.

Cutaneous metastatic cribriform adenocarcinoma. This was CK7 positive, CK 20 negative, ER positive, PR positive. Further work up confirmed the breast as the origin.
