F22, Purpuric plaques, some with areas of impending necrosis, over both lower limbs of 1 week duration.

Thrombotic vasculopathy. ? Antiphopholipid antibody syndrome ? purpura fulminans.

Agree with thrombotic vasculopathy histologically, there are a whole host of conditions leading to this pattern. It is acute if the stated duration is correct and given her age APS or sepsis with purpura fulminans are possible DDx but needs integration of CPC data to hopefully finalise the diagnosis.

Agree with thrombotic vasculopathy, acute stage. ? Poorly controlled Ulcerative Colitis; ? Covid-19 ? Drugs/status post-vax covid19.

Yes! this was thrombotic vasculopathy. Onset was preceded by fever and COVID-19 infection was suspected. Unfortunately patient was lost to follow up.
