Lobular panniculitis with a lot many neutrophils and acid fast bacilli in the cyst- like spaces created by damage to the adipocytes are the salient features. With the history of kwashiorkor which causes immune deficiency, it's infective panniculitis due to atypical mycobacteria in the setting of immunodeficiency.

Since the incubation period of leprosy is 2 to 10 yrs, it's hardly ever seen in infancy.

This was confirmed by culture to be disseminated BCG infection. Culture revealed mycobacterium bovis.
