Amazing case! Did the patient apply topical corticosteroids on the lesions?

I think that is the most likely explanation for the atrophy, which clinicallly is often peri-follicular, as might be expected.  There is visible alteration in the quality of the collagen in the upper dermis, with a washed-out appearance and telangiectasiae, probably steroid-induced, but not to the same extent as might be expected in lichen sclerosus.
