Large cell transformation in the clinical setting of MF. In the absence of classic clinical setting of MF, then needs to be differentiated from other high grade T-cell such as ALCL and B-cell lymphomas on IHC.

Tumour stage mycosis fungoides with large cell transformation (greater than 25% and microscopic nodules). Blast-like appearance (never understood the 4 x diameter of small lymphocyte as this is mega large).  CD30 positivity would be of interest (usually CD30+).  Need compatible clinical history for mycosis fungoides.

Nodular and wedge shaped

C-ALCL vs systemic to skin (LyP, type C)?

If p/h/o MF then Sezary/MF transformation if clinical fits
