77 year old male with extensive pustular lesions, arms, legs, and trunk.

Two dermal papillae appear to be stuffed with neutrophils. There is a polymorphous infiltrate in the dermis. I think that considering the clinical description, it could be a case of histiocytoid Sweet syndrome associated with hematological malignancy.

dermal papilla with neutrophils some amount of vascular damage seen

also to consider SLE

other possibility - intraepidermal neutrophilic dermatosis-IgA pemphigus

lab investigations like complete blood counts/ANA/DIF data needed

Patient had positive linear IgA deposition on DIF. Diagnosis-medication related linear IgA Bullous dermatosis.
