? Cutaneous Mastocytosis

The apoptotic keratinocyte high up in the epidermis with an adjoining lymphocyte (? satellite cell necrosis) suggests a drug rash or acute graft versus host disease. Since the rash is generalized, it's unlikely to be adult onset Still's disease which is another differential. Clinical history and correlation needed.

agree regarding the the possibility of GVHD, we need clinical information

Sezary syndrome vs GvHD. IHC + clinical correlation required.

Interface drug eruption. No history of transplant. Blood work normal without Sezary cells.

Please Uma, what’s the 4022 diagnosis? 

Despite the age it looks like Mastocytosis. Can we see Toluidine blue or Giemsa?
