LSC with few enlarged melanocytes? Otherwise does not look specific. Although I always worry this will be something like dVIN that I miss.

In view of single interstitial lymphocytes in-between thickened collagen fibers, which serves as clue, I will keep a possibility of hypertrophic lichen sclerosus sans sclerosis. Clinical correlation will help for distinction between LSC and Lichen sclerosus in such cases.

Hypertrophic LS. In these cases the possibility of early dVIN should always be suspected and clinicians advised accordingly. Clinician plans to follow closely due to patient's age and repeat biopsy.
