Intraepidermal acantholysis, not a clean suprabasal cleft. Things to exclude: HSV, mucosal Grover's and Pemphigus (vulgaris); Need clinical and immunofluorescence data.

I am favoring pemphigus vulgaris. However definitive diagnosis is subject to clinical correlation and DIF.  How common or uncommon is mucosal involvement in Grover's disease, not aware of mucosal Grover's disease.

Rare - like case report rare.

Suprabasal cleft with lots of neutrophils and few eosinophils. I see only one histologic pattern (suprabasal acantholysis) but no corp ronds nor grain formation. I favour P Vulgaris.
