Will do Perl stain to check hemosiderin for granulomatous PPD. Differential could also include lichen scrofulsorum / tuberculide.

non-necrotising granulomatous inflammation with surrounding lymphoplasmacytic infiltrate... ? rule out infection (Tb, atypical mycobacteria, fungi) with Zn, Wade fite, DPAS, and grocott.

I have two question: can Lichen Nitidus have confluent papules? Is the age so weird for LN?

Lichenoid Granulomatous reaction pattern. The 1st thing to consider is Lupus Vulgaris/ Primary Inocoulation TB. Then can consider Sarcoidosis and the subcutaneous mycoses.

By CPC, this was concluded as lichenoid sarcoidosis.
