Looks like nodular-micronodular BCC with intravascular tumour deposits which appear to have clear cell change. Not common but rare case of BCC with intravascular invasion have been reported. 

Basal cell carcinoma with clear cell change is the first impression. Embolization of the vessels by clear cells is rather interesting. Trichilemmal carcinoma is a differential diagnosis.

Yes, this is a recent case of BCC with vascular invasion.

This is very uncommon in my experience.

And of course, it is difficult to know of the true clinical significance.... As it happened this particular patient is under regular follow-up for other lesions. But it is difficult to know whether such a patient needs to be followed up if this is an isolated finding. Probably it is an incidental finding and does not indicate a significant metastatic risk.
