88 year old male with posterior scalp lesion. Separately submitted margins are assessed as frozen
section specimens.

Malignant chondroid syringoma. CK7, CEA, CK 20, p63 stains for confirmation and to rule out a metastasis.

Hard to tell! May be margin involvement of BCC. ? Perineural invasion..

Agree challeging. Looks like perineural invasion in FS. Quality not great due to coagulation and freezing artefact. Would be best to convert to paraffin and do IHC for BerEP4, EMA and S100. Odd that the cases don't have feedback.

An extremely difficult case; these foci were missed on FS examination. Patient with diagnosis of microcystic adnexal carcinoma. A reminder of the relative subtlety of this entity and difficulty with FS assessments.
