Acral nevus. The degree of atypia is accountable for nevus of special site.

Dear All,

Thank you for your comments. These are difficult cases. Find below our report;

 

Microscopic Description :
Acral skin with an asymmetrical compound melanocytic lesion. There are epidermal nests with cytological atypia and lentiginous proliferation. The nests have epithelioid cells and there is a vague “spitzoid” feel. There is scattered hyperpigmentation, cytological atypia is mild with no conspicuous pleomorphism and I have seen no mitoses. The dermis shows vascular proliferation and mild inflammation and no fibroblastic scar. The lesion lies 0.3 mm from the lateral resection margin.


The features are those of an inflamed acral naevus with some architectural ‘atypia’ including upward spread and vertical elimination of nests.
