26 year young woman from Russia, postpartum. Left proximal pretibial nodular lesion, rule out erythema nodosum.

Erythema nodosum leprosum. But clinical correlation is required for distinction from atypical mycobacteria which can have overlapping histopathological features.

Atypical mycobacterial infection is more likely. ENL will show foamy cell granulomas and fragmented mycobacteria.

I agree with you both!

I issued a diagnosis of non-tuberculous mycobacterial dermatitis and panniculitis and suggested the possibility of ENL versus atypical mycobacterial infection.

PCR was negative for M.leprae and wound culture grew M.chelonae.
