46 year female on antiretroviral therapy duration 4 months Numerous discrete papules with shiny centres and slight hyperpigmented rim present on extensor aspect of forearms and legs, lower back/buttock.

Thank you for your comments. 

The final diagnosis is:  Xanthoma disseminatum.

The differential diagnoses to consider are:

Cryptococcosis.
Interstitial granulomatous drug reaction.
HIV associated granuloma annulare.
Eruptive xanthoma.
